Accessible Information Standard

BACK TO MAIN INDEX

At 157 Medical Practice, we are committed to making sure all patients can access and understand the information they need to manage their health and care.
The Accessible Information Standard (AIS) is an NHS requirement that ensures people with a disability, impairment or sensory loss receive information in a way they can understand, and the communication support they need.

What this means for you

If you have any communication needs, we will:

  • Ask you how you prefer to receive information
  • Record your needs clearly on your medical record
  • Share this information with other NHS services involved in your care (where appropriate)
  • Provide information in a format that works for you

How we can help

We can offer support such as:

  • Information in large print, easy read, or alternative formats
  • Communicating with you by email, text message, or phone, depending on your preference
  • Arranging a British Sign Language (BSL) interpreter or other communication support
  • Helping carers or advocates receive information (with your consent)

Tell us what you need

If you have any communication or information needs, please let us know:

  • When you register with the practice
  • At your next appointment
  • By contacting reception

This will help us support you better.

Our commitment

We have an Accessible Information Standard Policy in place to ensure we meet NHS requirements and provide inclusive, patient‑centred care. If you would like a copy of this policy, please contact the practice.
